Enhanced Vision Coverage Available
To have the enhanced (glasses/contacts) vision coverage, participants and covered spouses must take or update the Health Risk Assessment (HealthQuotient) in WebMD through www.gbophb.org by December 31, 2007. Each person must set up a username and password under their own social security number to show up as having taken the survey. If you have forgotten either part of your login information, please click “forgot…” and choose #3 “Forgot my username or password.” You may update this tool at any time during the year; annual updates are required to maintain this extra vision coverage in the next calendar year.
If you (or your covered spouse) do not participate in the survey, you have coverage for an annual comprehensive vision exam for $10 with a VSP provider. Find a doctor at www.vsp.com.
