Please complete the dCOM Information Form. Once completed you will be prompted to select your time to meet with dCOM.

If you are seeking certified candidacy, you must first attend the Conference Candidacy Summit and complete 6 months of mentoring.
  1. Complete the dCOM Information form
  2. After submitting the dCOM Information form you will receive a pop-up window confirming submission and redirect you to schedule your meeting with dCOM
  3. Signup for a morning hour time slot – if you have a conflict with the times available, please send email to northeast@ngumc.net Immediately.
  4. Coordinate with your home church pastor to ensure that “Declaration of Candidacy and Charge Conference Recommendation (Form 104)” is submitted during Church Charge Conference. Form can be completed via paper and uploaded to District OR through Data Services under Charge Conference.
  5. Three weeks before your meeting with dCOM, you will receive a link to upload required paperwork for dCOM to review. Review the Certified Candidate checklist (attached) for required documents and upload documents two (2) weeks before your scheduled meeting.
    1. The Criminal Background check was completed before Candidacy Summit and a copy will be placed in your folder for you by the district
    2. If you need a notary, please contact northeast@ngumc.net to coordinate a time for notary. I am typically in the office on Mondays and Wednesdays but scheduling ahead of time will assist me in having my notary seal with me.

Local Pastors, Returning Certified Candidates, Supply Pastors, and Pastors of Other Faith:

All licensed pastors and certified candidates must come before dCOM on an annual basis to renew their license. Please read and follow each step carefully and contact me if you have any questions or concerns.

  1. Complete the dCOM Information form. Be sure to take the necessary time to answer the last three questions thoughtfully and meaningfully. This is an opportunity for you to share an update with dCOM about what has transpired in the last year in your personal life and ministry. The more information you can provide to dCOM the better prepared they can be for your time together.
  2. After submitting the dCOM Information form you will receive a pop-up window confirming submission and redirect you to schedule your meeting with dCOM
  3. Signup for an afternoon 30-minute slot – the earlier you signup, the better options for getting a preferred time.
  4. Two weeks before your meeting with dCOM, you will receive a link to upload required paperwork for dCOM to review. Review the list below and upload documents one week before your scheduled meeting.

  • Current credit score
  • Updated Course of Study or Seminary transcript (minimum of 2 COS per year until complete and at least part time enrollment at seminary until graduation)
  • Any updates from last dCOM meeting
  • If you have completed COS or Seminary, then you may upload your Continuing Education certificates
  • Current credit score
