Except for the I-9 Work Form, all forms should be provided to the Conference Benefits Office staff. Scanned copies only.

A Note Regarding Part-time Appointments - per the Annual Conference, clergy who are appointed 1/2 time or greater are eligible to receive benefits under CRSP. The 1/2 time or greater is determined by the salary paid to the clergy person (as per Wespath rules) in relation to the full-time compensation as approved by the Annual Conference. Please refer to Compensation Levels for Determining CRSP Eligibility for Part-time Clergy.
Forms Required Regardless of Appointment
- I-9 Work Form- filled out for and with your employer - provide to the church (do not send to Benefits) by July 1
Additional Forms for Existing Full Time Elders, Deacons, Provisional Elders, Provisional Deacons and Local Pastors
- Appointment Form –already appointed in N GA-if change necessary
- UMPIP Contribution Election Form- participation is optional, clergy who choose to update their election based on a change in appointment may do so using this form; however, the contribution from the prior appointment will roll over to the new appointment. If clergy choose to change their election, this form must be provided to Benefits by July 1

Additional Forms for Existing Part Time Elders, Deacons, Provisional Elders, Provisional Deacons and Local Pastors
- Appointment Form - DS fills out and brings to Appointive Cabinet meeting
- UMPIP Contribution Election Form- participation is optional, clergy who choose to update their election based on a change in appointment using this form; however, the contribution from the prior appointment will roll over. If clergy choose to change their election, this form must be provided to Benefits by July 1
- Pension Waiver- for those choosing to waive CRSP- the original notarized waiver must be received by the Benefits Office within 60 days of the beginning of the appointment in order to waive the pension expense.
- If clergy have waived pension at a prior appointment, this waiver does not extend to a new appointment. A new waiver is required for each appointment.

- Pre-Appointment Form 1st Time- DS fills out and brings to Appointive Cabinet meeting
- First Time Pension Enrollment form due to Conference Benefits by June 3
- Pension Waiver for those choosing to waive CRSP- the original notarized waiver must be received by the Benefits Office within 60 days of the beginning of the appointment in order to waive the pension expense.
